Appointments
Call 952-476-4537 (952- 4 Smiles) to set up an appointment. For your convenience you may download your patient information and health history to complete and bring with you to your appointment.

Insurance and Billing

  • Full payment is due at the time of service unless arrangements have been made prior to appointment.
  • A 5% bookkeeping savings will be offered for payment in full at the time of treatment. We accept cash, check or credit card (Visa, Mastercard, or Discover).
  • For crown and bridge treatment, we require 1/3 down at the crown preparation appointment.
  • Payment arrangements available: 3 consecutive monthly payments, the first one due at your first appointment.
  • Accounts must be paid in full before scheduling of next phase of treatment.
  • Outside financing programs are available.
